Application Form : The application form , along with necessary instructions, an acknowledgement card and envelope for sending application is obtainable from the office of Haryana Public Service Commission, Bays No.1-10 Block-B, Sector -4 Pancukula by making a payment at following rates:

Submission of Application Form : Candidates should ensure that the duly filled application forms for admission to the Preliminary Examination must reach Commission's office at Bays No 1-10 , Block-B, Sector -4, panchukula by post or by personal delivery by close of office working on or before 27/12/2011.
